As an On-Call LVN/LPN, you will: Provide direct nursing care and administer medications in accordance with physician orders and individualized care plans. Monitor resident conditions and report changes promptly to physicians, families, and the interdisciplinary team. Assist with admissions, discharges, treatments, and resident assessments. Support care planning and coordinate with healthcare professionals to ensure optimal resident outcomes. Supervise and guide Certified Nursing Assistants (CNAs) while promoting high standards of care. Leadership & Documentation Direct and oversee nursing activities during assigned shifts. Maintain accurate and timely documentation, including charting, physician orders, treatment records, and care updates. Complete incident reports and participate in quality assurance initiatives. Compliance & Safety Follow all federal, state, and facility regulations, including HIPAA and infection prevention standards. Ensure proper medication administration, narcotic accountability, and treatment documentation. Promote a safe, clean, and comfortable environment for residents, visitors, and staff. Participate in required in-service training, facility surveys, and safety programs. Qualifications Current, active, and unrestricted California LVN/LPN license. Graduate of an accredited LVN/LPN nursing program. Experience in skilled nursing, post-acute care, or long-term care preferred. Strong clinical judgment, communication, and organizational skills.
