Empower patients with health education, consult with community support specialists, manage hospital admissions and discharge processes, and guide patients through diagnoses, treatments, and medications. Position Perks & Benefits: • $5,000 Sign-On Bonus • Paid time off - 29 days per year including vacation & holiday pay • Mileage reimbursement - company paid for work functions requiring travel • Top-notch training - initial, ongoing, comprehensive, and supportive • Career mobility - advancement opportunities/promoting from within • Welcoming, warm, supportive - a work culture & environment that promotes your well-being, values you as human being, and encourages your health and happiness Key Responsibilities: • Develop wellness and preventive initiatives. • Spearhead health education classes, tapping into profound insights on complex medical conditions. • Contribute to the preliminary treatment scheme creation for all Healthcare Home participants. • Team up with professionals to design a targeted treatment strategy comprising healthcare objectives for individuals afflicted by dual chronic diseases. • Advise Community Support Specialists on patient-identified health complications. • Streamline hospital and medical facility admission and discharge proceedings for Healthcare Home beneficiaries. • Coach patients on complex health conditions, discussing medical diagnoses, therapies, and prescriptions. • Execute DMH Net Health technology projects and endeavors, such as CyberAccess and metabolic screening. • Track and relay performance metrics and triumphs using refined information systems engineered for nurses.
