What You’ll Do: Provide palliative care through evaluation and assessment of health history, diagnosing medical conditions. Develop individualized plans of care. Complete face to face assessments and prescribe medications or treatments. Collaborate with the interdisciplinary team. What You Bring: Active NP certification (state-licensed). Valid driver’s license, reliable transportation, and current auto insurance.
