Job Description Summary **Candidates in the Columbus region strongly preferred** The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services. You will ensure timely access to medically appropriate care while promoting continuity, quality outcomes, and a positive patient experience. We are seeking a skilled and patient-focused Case Manager (RN) to join our growing team. Compensation Range: $73,343.00 to $110,014.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting.
