Physicians Choice Home Health is hiring a full-time or part-time/per diem Occupational Therapist to join our Home Healthcare Team and see patients in Southern California's South Bay Area. In this role, you will provide high-quality occupational therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will supervise a Certified Occupational Therapy Assistant (COTA) and work with a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care.
What You'll Do
- Provide in-home occupational therapy evaluations and treatments according to the physician’s plan of care
- Assess patients’ strength, coordination, mobility, and ability to function safely at home
- Develop and update individualized treatment plans based on patient goals and progress
- Help patients improve independence with daily activities such as dressing, bathing, mobility, and home safety
- Educate patients and caregivers on exercises, adaptive techniques, and safe movement
- Document evaluations, visits, and progress in WellSky/Kinnser in a timely manner
- Communicate patient progress, concerns, and changes in condition to the care team and physician
- Supervise COTAs in accordance with state guidelines
- Participate in care planning, case conferences, and discharge planning
Requirements
- Graduate of an accredited Occupational Therapy program (Master’s or Doctorate in Occupational Therapy)
- Active California Occupational Therapist license
- Current CPR/BLS certification
- Reliable transportation and willingness to travel to patient homes
- Home Healthcare experience
- Experience documenting in the Kinnser/Wellsky EMR preferred, but not required
