Develop individualized treatment plans and coordinate discharge goals. Educate patients and caregivers and connect them with support resources. Collaborate with care teams and advocate for patient needs. Provide age-appropriate care and follow applicable regulations. Apply clinical knowledge, sound judgment, and basic computer skills. Settings may include inpatient, outpatient, long-term care, community-based care, and home health. Perform other duties as assigned.
