Based in Melbourne CBD, the Care Manager manages the client relationship through the delivery of excellent customer service and the implementation of client care initiatives. Managing a caseload of self-managed clients in a virtual capacity - no home visits required - you are responsible for ensuring that all service delivery and compliance reporting requirements are met. You will also work collaboratively with various teams to ensure that services delivered are of a high quality and offer an excellent client and workforce experience.
Day to day you will:
- Develop care plans that support the delivery of quality outcomes for clients in line with their funding
- Actively promote Homecare My Way services to meet growth targets
- Complete all funding and debt management activities
- Ensure all client documentation and case notes are up to date, complete and of a high standard
- Support the business development and onboarding function, including the assessment and suitability to provide services in line with Homecare My Ways’ related policies and procedures
Let's talk about you!
A successful candidate will demonstrate a strong understanding of Aged Care and/or other health and community services. You will bring experience from working in customer focused environments and have excellent time management and prioritisation skills, exceptional communication skills, both written and verbal - imperative to working in this 'virtual' office-based role. You will be able to demonstrate a willingness to learn and adapt to changing environments/ markets. Candidates with a background in nursing are highly desirable.
