The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes. Qualifications Graduate of NLN Accredited School of Nursing BSN Preferred or Masters Degree from an accredited school of Social Work 1-3 years experience in an acute care hospital Ability to read, write, and communicate in English. Strong clinical, organizational, and interpersonal skills. Current New York State Licensure as a Registered Professional Nurse.
