Job Description: Ongoing training and development Friendly and dynamic team Full time permanent opportunity OR Full time fixed term until 25/6/27 (will consider part time) About the Role As a Clinical Assessor, you will work as part of a multidisciplinary team to deliver timely, comprehensive and holistic assessments of frail older people in community and hospital settings. You will actively involve clients and carers in developing support plans tailored to individual needs and coordinate referrals to relevant service providers to support positive outcomes. Conduct holistic clinical and, where required, non-clinical assessments of older people in community and hospital settings. Review support plans, undertake home risk screening and complete My Aged Care triage as required. Involve clients and carers in assessment and care planning, developing short- and long-term plans that address individual health, physical, social and functional needs. Participate in team case conferences and implement agreed recommendations to support quality care and service delivery. Liaise with clients, carers, referrers and internal and external service providers to coordinate care and optimise client outcomes.
