Job Description: Clinical Pharmacist – Hospital in the Home & Residential Inreach Part Time, Fixed Term – 16 hours per week until 30 June 2027 Deliver specialist clinical pharmacy care to patients in their homes and residential care settings Join a collaborative multidisciplinary team within one of Australia’s leading health services About the Role An exciting opportunity exists for an experienced Clinical Pharmacist to join the Hospital in the Home (HITH) and Residential Inreach (RIR) services at St Vincent’s Hospital Melbourne. Working alongside the Senior Hospital in the Home Pharmacist, you will provide high-quality clinical pharmacy services to patients receiving acute care within their own homes and residential care facilities. This varied role offers the opportunity to work across hospital and community settings, supporting the safe, effective and appropriate use of medicines while working closely with patients, carers and multidisciplinary healthcare teams. As part of the role, you will travel to patients’ homes and residential facilities to provide clinical pharmacy services and support continuity of care outside the traditional hospital environment. Your responsibilities will include: Providing clinical pharmacy services to HITH and Residential Inreach patients in their homes and residential care facilities Obtaining comprehensive and accurate medication histories and undertaking medication reconciliation Reviewing medication therapy and providing recommendations to support safe and optimal medication use Providing medication counselling and education to patients, families and carers Promoting the safe, timely and cost-effective use of medicines Collaborating closely with medical, nursing, pharmacy and other healthcare professionals Liaising with community pharmacies, general practitioners and other external healthcare providers as required Supporting safe transitions of care between hospital, home and residential care settings Maintaining accurate and timely clinical documentation Salary: Classifications for this position will range from SX2–SX5 ($94,478.80–$106,906.80 per annum, pro rata for part time). Your Contribution You are an experienced hospital Pharmacist with strong clinical judgement, excellent communication skills and the confidence to work both independently and collaboratively within a multidisciplinary environment.
