The position is located at the Region 2 Field Office in Augusta, Georgia.  Must be a highly organized individual, as this position will require a significant amount of tracking and follow-up for many different job functions  Requires completion of a significant number of Needs Assessments for new individuals as well as annual renewals for individuals on the Planning List  Requires telephone support to individuals and families on the Planning List, and bringing resolution to phone calls and messages  Requires reporting Planning List changes and concerns to Planning List Supervisor  Make and resolve referrals to outside agencies for non-waiver services  Other supports for Planning List as needed. Qualifications: Bachelor’s degree, Human Services field preferred. Experience working with individuals with intellectual/developmental disabilities is strongly preferred. Must have at least an intermediate level of computer knowledge, Office knowledge preferred. Must report to Region Two Field Office in Augusta, Georgia.
