Job Duties: This position is for a Geisinger provider working within a contracted skilled nursing facility. It requires an experienced advanced practitioner with prior clinical experience in skilled nursing, family medicine, internal medicine, or intensive care. The advanced practitioner (AP) provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.
