The RN Case Manager provides coordination of Early Intervention and Transitional Case Management services, including discharge planning and linkage to community-based services, to address the needs of HIV positive clients upon release from the Maricopa County jail system. Work With Us | Maricopa County, AZ We Require Two years of Registered Nurse experience Valid unrestricted license as a Registered Nurse issued by the Arizona Board of Nursing (AZBN) Bachelor’s degree in nursing from an accredited institution An equivalent combination of post-secondary education and/or nursing experience may substitute for the minimum qualification on a year-for-year basis We Also Value Bilingual skills in English/Spanish Experience with interviewing techniques, conducting complex nursing assessments, and providing client care Case management experience working with individuals with communicable diseases Job Contributions Works with CHS staff to identify clients in the jail system who may qualify for transitional case management services Reviews client's possible release status, using the jail’s data systems and medical records Provides transitional case management services, including discharge planning and linkage to community-based HIV care and services, to clients released from jail to the community; completes an HIV nursing assessment with medical, dental, substance abuse, mental health, and social histories to facilitate linkage to appropriate referrals in the community upon discharge; works with clients to complete an individualized discharge plan; and initiates referrals to community outreach and case management agencies Provides transitional case management services, including, but not limited to, providing counseling for incarcerated HIV clients about management of a chronic condition; and providing health maintenance information and treatment adherence counseling, including nutrition, monitoring of lab results, medication regimen, and liver health Reviews and monitors databases to identify HIV positive clients, coordinates care, and collects data Documents case management, health education, and care coordination in medical records Coordinates with CHS medical providers and/or jail staff to meet with patients during scheduled clinic hours to assess needs and provide case management services for HIV positive clients Maintains documents, CAREWare information, and Public Health surveillance systems to facilitate referrals and program audits in compliance with HIPAA, Ryan White, and CHS requirements Working Conditions Work is performed primarily in a correctional setting, with some office time required Must be able to participate in emergency response training as scheduled and be available to respond as directed in the event of a public health emergency Valid Arizona driver’s license is required upon hire Travel will be required to attend conferences, training, and possible varying work locations Ability to exert up to 20 lbs. occasionally, 10 lbs. frequently, or negligible amounts constantly OR requires walking or standing to a significant degree Ability to work with a variety of programs and administrations including Arizona Department of Health Services (ADHS), Maricopa County Department of Public Health (MCDPH), Correctional Health Services (CHS), and the Maricopa County Sheriff’s Office Ability to draw out critical medical and personal information from an individual in a limited amount of time Must secure and maintain an Arizona Department of Public Safety Level 1 Fingerprint Clearance Card and complete a background check Within one month of hire, all employees will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test Selection Procedure Consideration will only be given to candidates who submit online applications Candidates will be contacted primarily through email and their Workday online application profile Must pass a pre-employment background and/or fingerprint investigation as required by statute or policy, including drug and alcohol testing requirements for positions designated as safety-sensitive Maricopa County is an equal opportunity employer and is committed to providing equal employment opportunities to all applicants.
