Our unique campus features 12 cottage-style homes (12 residents each), each with its own kitchen, along with gardens and patios for residents and staff to enjoy. Key Responsibilities Provide direct patient care in accordance with care plans Administer medications and treatments as prescribed Monitor resident conditions and report changes to the clinical team Support and supervise CNAs to ensure high-quality care delivery Maintain accurate and timely clinical documentation Promote a safe, clean, and respectful environment Collaborate with interdisciplinary team members Qualifications Active, unencumbered LPN license in South Carolina (required) Current CPR certification required Must maintain all continuing education and remain in good standing with the State Board of Nursing Experience with PointClickCare (PCC) preferred Compassion for an underserved population Ability to remain physically active throughout shift (walking between cottages required) Why Join Brushy Creek Post Acute?
