What You’ll Be Doing
The APP (NP/PA) will be responsible for providing comprehensive care to patients in a single skilled nursing facility. This is a Monday–Friday role with no night or weekend call, designed for clinicians who want to focus on delivering high-quality, efficient, and patient-centered care. You will collaborate with the facility staff, physicians, and other members of the healthcare team to ensure timely, effective, and coordinated care.
Key Responsibilities
- Provide direct medical care to patients in the assigned facility, including assessment, diagnosis, treatment, and follow-up.
- Manage acute and chronic conditions, focusing on high-quality, efficient care delivery.
- Coordinate with facility staff, specialists, and families to develop and update individualized care plans.
- Support transitions of care, including post-hospital evaluations, medication reconciliation, and patient/family education.
- Maintain thorough, timely, and compliant documentation of patient encounters and care plans.
- Participate in quality initiatives and process improvements that enhance patient outcomes and efficiency.
- Collaborate with the interdisciplinary team to ensure continuity of care and adherence to evidence-based protocols.
Skills & Abilities
- Strong clinical knowledge of adult and geriatric medicine, with expertise in chronic disease management.
- Excellent communication and collaboration skills with patients, families, and facility staff.
- Ability to manage a patient panel efficiently while maintaining a high standard of care.
- Strong organizational and documentation skills.
- Commitment to patient-centered, evidence-based practice.
Qualifications
- Current Nurse Practitioner or Physician Assistant license in the state of Georgia.
- Certification in Internal Medicine, Geriatrics, or Family Practice required.
- Must have an unrestricted DEA license
- Minimum 1–2 years of NP experience preferred; experience in post-acute, SNF, or geriatrics strongly preferred.
- Ability to work independently in a facility setting with physician support.
- Familiarity with EHR systems and clinical documentation best practices.
